In this example we’re going to deal with requesting a certificate following an employees absence due to sickness.
We need a certificate for all employees who have been absent for at least 3 days, that are now back at work and haven’t yet provided a certificate.
The certificate needs to be provided within 2 weeks of request.
If we do not receive the certificate within 2 weeks, we’ll email the employees line manager requesting the documentation within 2 weeks.
If we don’t receive the certificate within 2 weeks we’ll send a letter to the employee threatening suspension.
If we still don’t receive the certificate within 2 weeks, the employee will be suspended.
If and when the certificate is returned, we’ll send an email of thanks to the employee.
